改良陰莖延長術的臨床應用

[摘要] 目的 采用改良陰莖延長術治療陰莖短小以及陰莖基本正常合并包皮過長。方法 在恥骨下區陰莖根部作M形切口，切斷陰莖淺懸韌帶和部分深懸韌帶，剝離皮瓣修復恥骨下區缺損。修剪包皮，切緣連續縫合。結果 24例患者術后陰莖頭外露，陰莖皮膚滑動好，切緣平齊，恥骨下區無凹陷，陰莖自然顯露長度平均為(6.9 ±1.5)cm，勃起時平均為(10.9 ±1.2)cm。隨訪5-8個月，效果滿意。結論 本法術后陰莖延長，形態更趨于自然美觀。

[Abstract] Objective To investigate an ideal operation of penile lengthening for penile short cases and normal but redundant prepuce cases. Methods After a ‘M’ incision was made under the public bone， the superficial ligament and part of the deep ligament were cut ，and the defect under the public bone were repaired . Then the prepuce was trimed and continuous suture was made at the cutting edge. Then cut the foreskin and sutured continually. Results Of these 24 cases ， balanus reveal greatly， the skin of coles glide well， the cutting edge is fluent and no hollow exisits under the punlic bone and the average length coles are about (6.9±1.5)cm， and after erection the average length of the penile was(10.9 ±1.2)cm. The result are well in the 5-8 month’s follow up. Conclusion The modified penile lengthening operation is more effective and nature than conventional.

陰莖的尺度影響著很多男性的生活，美國的一項統計表明，約2 %男性認為自己的陰莖不足以完成性交[1]。由于人們對生殖器關注程度的加大，相當部分的患者僅僅是為了滿足心理上的需要，而使陰莖延長術由重建手術發展成一種整形美容手術。我院在2004年3月-2008年8月采用改良陰莖延長術治療陰莖短小以及陰莖基本正常合并包皮過長24例，效果滿意，現報告如下。

1資料與方法

1. 1一般資料

本組24例合并包皮過長，年齡19-40歲，平均27歲。室溫25℃左右下測量陰莖常態長度為﹙3.8 ±1.2﹚cm;勃起狀態長度平均為﹙7.1 ±1.5﹚cm。均迫切要求手術。

1. 2手術方法

于恥骨下區陰莖根部作M形切口，盡量避免損傷皮下淺靜脈， 向前下方牽引陰莖，術者用右食指靠恥骨往下鈍性分離，緊靠恥骨聯合切斷陰莖淺懸韌帶及部分深懸韌帶，以達顯露陰莖背深靜脈為限。剝離恥骨下區兩側的脂肪瓣連同恥骨弓兩側結締組織和脂肪組織向中央用4號絲線拉攏縫合，消除恥骨下空腔，在陰莖淺筋膜層面剝離筋膜組織瓣，向根部牽拉。……
