靜脈腔內激光聯合小切口高位結扎治療單純下肢靜脈曲張

[摘要] 目的 探討靜脈腔內激光治療(Endovenous laser therapy， EVLT)聯合小切口高位結扎在治療單純性下肢靜脈曲張中的療效。方法 回顧2005年10月-2009年6月間我院行EVLT聯合高位結扎手術的73例共90條患肢的臨床資料。結果 除1例局部治療遺漏，經再次治療外，其余患者靜脈曲張均一次治愈，所有病例隨訪3-44個月無一例復發。結論 EVLT聯合小切口高位結扎術療效可靠，是目前治療單純下肢靜脈曲張的首選微創手術方法。

[Abstract] ObjectiveDiscussion on the effect of EVLT combined high ligation in the treatment of simple lower extremity varicose vein. Methods Review of the clinical data on 73 cases ，total 90 affected legs operated by EVLT combined high ligation done at our hospital during the period from October 2005 to June 2009. ResultsAll the varicose veins have been cured and disappeared except for 1 casein which a single varicose vein has been omitted for local therapy and undergone byanother operation later on，and no recurrences during the follow up of 3 to 44 months. Conclusion EVLT combined high ligation is effective and reliable，and is preferred mino-invasive operation for simple lower extremity varicose vein at present.

單純性下肢靜脈曲張為常見血管外科疾病，我院自2005年10月-2009年6月期間，采用靜脈腔內激光治療(Endovenous laser therapy，EVLT)聯合小切口大隱靜脈高位結扎治療單純性下肢靜脈曲張73例，共90條患肢，取得了可靠的療效，并發癥少、美觀、住院時間短、恢復快。現報告如下:

1臨床資料

1.1一般資料

本組73例患者，其中男45例，女28例;左下肢33例，右下肢23例，雙下肢17例，共90條患肢。年齡25-67歲，平均47.5歲;病程2-35年，平均23.5年。其中硬化劑注射治療后復發7例，單純EVLT后復發3例。病人按CEAP分類[1]，C220例、C39例、C442例、C51例、C61例。全組病人均作患病下肢血管多普勒超聲檢查或血管造影等檢查，排除深靜脈瓣膜功能不全Ⅲ級及以上患者;排除深靜脈血栓形成后遺癥，動-靜脈瘺，髂靜脈閉塞綜合征，Budd-chiari綜合征和靜脈畸形骨肥大綜合征等;并了解交通瓣膜功能情況。

1.2手術方法

本組均于術前標記患肢曲張靜脈團和內踝前上方大隱靜脈走向。選硬膜外麻醉。激光治療儀為國產半導體激光手術刀，型號:HOP-100。……
